NHS prescriptions in England: £9.90 an item and how to pay less

In England an NHS prescription costs a flat £9.90 per item, whatever the medicine actually costs. Two medicines a month comes to almost £240 a year. Yet a very large share of people are entitled to pay nothing at all, or to pay roughly a third of that, and simply do not know it. Here is who is exempt and how to cut the cost legally.

How much does an NHS prescription cost in 2026

£9.90 per item. The charge is not per appointment or per box but per named medicine on the form: three different medicines means three charges of £9.90. The charge has been frozen since April 2025, when prepayment certificate prices were fixed too.

What is a prepayment certificate and when is it worth it

A Prescription Prepayment Certificate (PPC) works like a season ticket: you pay once and all your prescriptions are covered for the period. The prices are £32.05 for three months and £114.50 for twelve.

The break-even point is simple. A PPC saves money if you need more than 3 items in 3 months, or more than 11 items in 12 months. In other words, one repeat medicine a month already makes the annual certificate worthwhile.

The 12-month certificate can be paid in ten monthly Direct Debit instalments. That option is not available at a pharmacy counter — only when you buy online.

Is there a separate certificate for HRT

Yes. The HRT PPC costs £19.80 and lasts 12 months, covering certain hormone replacement therapy medicines. It is a separate document and cheaper than the standard annual PPC.

How to buy a certificate

Online through the NHSBSA website, or in person at some pharmacies. The helpline is 0300 330 1341. You can also ask NHSBSA to switch you to a digital PPC, which is sent to you by email.

Who gets free NHS prescriptions

Under NHSBSA rules you pay nothing if you are under 16; aged 16 to 18 and in full-time education; or aged 60 or over. If your date of birth is printed electronically on the prescription, you do not have to prove your age.

Prescriptions are also free if you are pregnant or have had a baby in the last 12 months and hold a valid maternity exemption certificate.

On benefits: Pension Credit Guarantee Credit qualifies (Savings Credit alone does not), as does Universal Credit if your take-home pay in the last assessment period was under the limit — tick box 'U' on the form, or box 'K' if there is no 'U'. New Style Jobseeker's Allowance and New Style Employment and Support Allowance do not give automatic entitlement; with those you apply to the NHS Low Income Scheme instead.

An HC2 certificate gives full help with health costs; an HC3 gives partial help and states how much you pay. A valid war pension exemption certificate also exempts you, but only for prescriptions linked to the accepted disability.

Contraceptives and medication given to hospital inpatients are always free.

Which conditions qualify for a medical exemption certificate

A five-year certificate is issued if you are being treated for cancer, the effects of cancer, or the effects of cancer treatment.

A ten-year certificate covers: a permanent fistula needing continuous surgical dressing or an appliance; a form of hypoadrenalism such as Addison's disease requiring essential substitution therapy; diabetes insipidus and other forms of hypopituitarism; diabetes mellitus, unless treated by diet alone; hypoparathyroidism; myasthenia gravis; myxoedema requiring hormone replacement; epilepsy requiring continuous anticonvulsive therapy; and a continuing physical disability meaning you cannot go out without another person's help.

Your GP gives you the application form. The certificate is posted within 10 working days of NHSBSA receiving the application and is backdated to one month before receipt. People aged 60 or over do not need to apply.

What if you paid when you were entitled to free prescriptions

Refunds are claimed on form FP57, which you must ask for at the pharmacy at the moment you pay — it cannot be issued later. Where a certificate is involved, it must cover the date you paid, and the claim must be made within three months of payment.

How long is the prescription itself valid?

Most prescriptions are valid for 6 months from the date a doctor or nurse signs them. For most controlled drugs, such as morphine, it is shorter: 28 days. A pharmacy will not dispense an expired prescription, and you will have to go back to the prescriber.

What about outside England?

The prescription charge is an English matter. NHS prescriptions in Scotland and Wales are free. For anyone who has moved to England from Edinburgh or Cardiff, the first pharmacy bill is an unpleasant surprise, so it is worth knowing in advance.
