NHS prescriptions in England: how a PPC cuts your medicine bill
A single prescription item in England costs £9.90; a 12-month PPC is £114.50 for any number of items. Here is when it pays off and how to buy one.
Thanks for £1
In England the NHS charges a fixed fee for each prescription item. If you take several medicines, or need them regularly, the yearly total adds up fast. That is what the Prescription Prepayment Certificate (PPC) is for: you pay once and then get your prescriptions with no further charge for the life of the certificate. Here is when it pays off and how to get one, based on NHSBSA.
What does a prescription and a PPC cost?
A single prescription item in England costs £9.90. A PPC comes in two lengths: 3 months for £32.05 and 12 months for £114.50. With the certificate you get as many prescription items as you need at no extra cost during that period.
When is a PPC worth it?
The rule is simple: a certificate saves you money if you need more than 3 items in 3 months, or more than 11 items in 12 months. A £114.50 annual PPC has effectively paid for itself by around the twelfth chargeable item, and everything after that in the year is free. If you take one or two medicines all the time, count your items over a quarter — the certificate is very likely cheaper than paying each time.
Can you pay in instalments?
Yes. A 12-month certificate can be paid in ten monthly Direct Debit instalments. That option is available when you buy online, but not at a pharmacy. A 3-month PPC is paid in full.
A separate certificate for HRT
Women prescribed hormone replacement therapy (HRT) have a dedicated certificate, the HRT PPC. It costs £19.80 and lasts 12 months, covering the relevant HRT medicines with no per-item charge.
How do you get a PPC?
You can buy a certificate three ways:
- online through the NHSBSA service (buy-prescription-prepayment-certificate.nhsbsa.nhs.uk), where instalments are also offered;
- by phone on 0300 330 1341 if you need extra help;
- at some pharmacies.
A certificate starts from a date you choose, so you can buy it in advance.
What if you do not live in England?
This matters: prescription charges only exist in England. In Scotland, Wales and Northern Ireland NHS prescriptions are free, so there is no need to buy a PPC there. Some groups in England also get free prescriptions — the full list of exemptions is on the NHSBSA pages.
